Personal Guarantee of Professionalism: Volunteers hereby agree to arrive on time (times assigned by position), scare to the best of their ability, not touch customers or use profanity in their act. Will not take, damage, or deface haunt property. Will not use their cell phones while acting. Will notify one of the crew if needing to take a break.
I hereby authorize Awaken Haunted Attraction (herein referred to as “Awaken”), to publish photographs and/or
video taken of me and my name and likeness, for use of Awaken print, online based video/photo, and marketing
materials, flyers, business cards, merchandise prints, billboards, artwork, and all other company publications.
I hereby release and hold harmless Awaken from any reasonable expectation of privacy or confidentiality associated with the imagery specified above. I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any kind associated with the taking, publication, or participation of Awaken marketing-materials or other Awaken publications.
Risk Indemnification: I hereby consent to volunteer at Awaken Haunted Attraction at my own risk, fully understanding the risks involved including possible uneven flooring, walkways containing debris, props containing potential hazards if misused, exposure to patrons including possible hazards like screws, nails or a malfunction on a prop. Limited vision due to fog and low lighting and strobe effects.
Risks including customer reactions, injuries to myself or other customers and volunteers due to reactions to staff
and actors may result in sprains, bumps, bruises, breaks, or other minor injuries. I take responsibility knowing my limitations. These risks and dangers may be caused by the negligence of the owners, directors, volunteers, employ-
ees, agents of Awaken, others in attendance, or other forces of nature including my participation in activities.
I hereby assume all risks and dangers as well as all responsibility for any losses, damages, injuries as a result of
participation and attendance. I have read posted rules and will hereby abide by them. I fully understand and an of
sound mind to hereby sign my consent to participate with full legal consent.
Liability Indemnification: I understand Awaken will under no circumstances take responsibility, legal or monetary, for liability involving injury, loss, or damage to yourself or any minors in attendance or their vehicles and belongings. I understand Awaken provides a reasonably safe environment for my entertainment and enjoyment but make no representation or warranty of any kind express or implied, regarding the safety of patrons.
I on behalf of myself hereby voluntarily agree to release, waive, discharge, hold harmless, and indemnify Awaken and its owners, directors, volunteers, employees, and agents of Awaken from any and all claims, actions or losses for bodily injury, property damage, wrongful death or otherwise harmful events which may arise out of my participation at Awaken that I may have previously, presently, or in the future participate(d) in attending and working at the haunted attraction.
APPLICANT CERTIFIES THAT THE ABOVE STATEMENTS ARE TRUE & ALL STATEMENTS ARE COMPLETE. Applicant understands that falsification is automatic grounds for dismissal. Applicants are subject to criminal background checks, references, previous employment or haunt verification with proof if requested and hereby give consent and agree to fully cooperate with that. This application is subject to approval by the owner and the owners agents.
